Lab Portal Access

To create a patient portal for Cleveland Heart Lab:

• Call Cleveland Heart Lab (CHL) at 1-866-358-9828 and select option 1.
• Tell the representative that you would like to set up a patient portal.
• Prepare to verify your name, date of birth, and doctor’s name (Charlotte Widick).
• CHL will guide you through setting up your user name and password.

To check your CHL results:

• Go to https://portal.clevelandheartlab.com/.
• Enter your user name and password.
• Click the “Sign In” button to view your lab results.

To create a patient portal for Quest Diagnostics:

• Go to https://myquest.questdiagnostics.com/web/home.
• Click the “Create Account” button.
• Fill out the information to create your account.
• Quest will send a confirmation email.
• When you get the email, click the link to verify your account.

To check your Quest results:

• Go to https://myquest.questdiagnostics.com/web/home.
• Click the “Sign In” link in the upper right hand corner.
• Enter your login information.
• Click the “Login” button to view your lab results.
